This Service Agreement ("Agreement") is made and entered into on this day

by and between

TGI Brook Healthcare, the healthcare facility providing residential and clinical care services (Hereinafter referred to as "Client")

and

A registered nursing services company operating under the ownership and management

of

Hereinafter referred to as "Contractor")

Purpose of Agreement

The purpose of this Agreement is to outline the terms under which

will provide professional nursing services to TGI Brook Healthcare. All services will be delivered by

Scope of Service

The

agrees to provide the following services:

• Skilled nursing care• Medication administration and monitoring• Clinical assessments and documentation• Coordination with facility staff and healthcare providers• Any additional nursing duties mutually agreed upon in writingAll services will be perfonned in accordance with state and federal regulations, professional nursing standards, and the policies of St. Patrick's Home.

Payment Terms

• TGI Brook Healthcare agrees to compensate

not the individual nurse, for all services rendered.

Payments will be made to:

• Payments will be issued twice a month on the 1st and on 16th of the month• No payments shall be made directly as an individual.
